Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love. As a Home Health LPN , you will: Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN. Implement individualized care plans in collaboration with the patient, family, and healthcare team.
