This role has two primary focuses: providing NP-level hospice care to enrolled pediatric patients and supporting a specialist pediatric palliative care consultation service. The vast majority of visits will take place in the patient’s home , with occasional visits to hospitalized pediatric patients as needed. Overview Provides advanced nursing clinical care to patients in the VNS Hospice Care program consistent with VNS Health philosophy and standards of nursing practice in relation to VNS Health goals and objectives and improves hospice utilization for hospice eligible patients. Works under general supervision. • Provides advanced nursing care and treatment to hospice patients in both the home and via telephonic visits, which includes assessment, diagnosis, prescribing medication, ordering and interpreting appropriate tests, and ordering treatments and DME. • Performs in-person pre-admission F2F visits when necessary, for certification of terminal illness by the Hospice Team Physicians and complete hospice admission to the Hospice Program when deemed hospice eligible.
