An ideal candidate is a team player, detail-oriented, has geriatrics experience, monitor and intervene on patient safety concerns, and carry out organizational protocols. Required Qualifications Current OR license Current CPR certification Palliative and/or hospice experience Palliative care nursing certificate - preferred Preferred Qualifications 3+ years of experience Responsibilities Conduct medical home visits on new and established Supportive Care patients, performing a hands-on exam at each visit. Conducts urgent/acute visits on all Supportive Care patients with the goal of keeping the patient out of the hospital to receive the best medical care at home....
