Job Description: Responsible to assess, plan, implement, evaluate, supervise and coordinate the home care and hospice care of patients in accordance with professional standards of nursing and policies and procedures of Essentia Health. Provides direct patient care to patients requiring care for medical, diagnostic and emergency conditions.
