Job Description: This position requires community health workers (CHW) to have a close understanding of the communities they serve. CHWs are specifically hired and trained to address behavioral and social determinants of health through patient-centered goal setting. CHWs reach beyond clinic walls to help patients meet their goals. CHWs creatively leverage patient, family, and community resources to address root causes of patient’s health concerns.
