About this Job: General Summary of Position Works in partnership with local communities to design implement and evaluate health promotion prevention and education programs. Requires Registered Dietitian licensure and bachelor's degree in nutrition/dietetics or other relevant degree and certification/licensure. Primary Duties and Responsibilities Contributes to the achievement of established department goals and objectives and adheres to department policies procedures quality standards and safety standards. Complies with governmental and accreditation regulations.Facilitate/teach (CDE) Diabetes Education and Self-Management Courses.Provide individual or group counseling within licensed scope of practice.Coordinate/facilitates bi-annual Diabetes Fairs.With hospital Case Management services establishes transition plans for patients who are being discharged from the hospital.Participates and demonstrates competency in Electronic Medical Record (EMR) initiatives.Demonstrates...
