Job Details Description Department/Location: Community Health Improvement 80 hours per pay period - benefit eligible Job Summary: Collaborates with members of the health care team to provide medical nutrition therapy. Identifies patients as high nutrition risk based on criteria outlined by MMH. Assesses nutrition needs & identifies nutrient requirements according to age & disease specific needs of patients. Develops & implements the nutrition care plan for all assigned patients. Fosters continuity of care among all phases across the continuum of care. Continuously evaluates & monitors effectiveness & outcomes of medical nutrition therapy intervention.
