In this role, you will complete nutrition assessments, develop and implement individualized care plans, and monitor patient progress across inpatient and outpatient settings. You will collaborate with physicians, nurses, and other clinicians to support complex medical conditions, educate patients and families on diet and lifestyle changes, ensure regulatory compliance, and contribute to performance improvement and community wellness initiatives.
