POSITION SUMMARY/RESPONSIBILITIES Assesses adult and pediatric pre-transplant and post-transplant patients' needs and assists with personal, financial, and environmental difficulties which prevent the patient from accessing medical care/transplant regimen. Counsels and assists patients and their families in receiving maximum benefits from third party payers for medical care. Participates in the evaluation process of transplant candidacy and living donor candidacy by completing psychosocial assessment and making recommendations for listing in transplant team conferences. Provides social services to all transplant patients both in hospital setting and in outpatient transplant clinic. Travels to out of town satellite clinics to perform psychosocial assessments as part of transplant evaluation and living donor evaluation.
