Apply Description The Case Management Coordinator is responsible for establishing, coordinating, and maintaining the process of inpatient admissions, pre-certifications, addressing insurance denials, collaborating with LLM and overseeing discharge planning and ensuring patients have a safe discharge plan. The Case Management Coordinator will collaborate with Intake Nurses, Utilization Review, Recreational Therapist and Social Workers, Patient/Family, Physicians, community resources and payers to ensure the patient’s progress and level of care is appropriately determined.
