Responsibilities: The Virtual Primary Care Family Medicine Advanced Practice Provider (Nurse Practitioner/Physician Assistant) provides comprehensive primary care to new and established patients for Jefferson Health via telehealth platform. This service will expand Jefferson Health primary care services to new and existing markets. S/he collaborates with, and is an integral member of, a multidisciplinary health care team. You ll play a critical role in building trusted relationships to make people healthier, helping bring flexible, comprehensive and personalized health evaluations right to people s front doors.
