The Advanced Practice Provider (APP) (Nurse Practitioner or Physician Assistant) works within an outpatient primary care clinic as part of a collaborative team, which operates under a comprehensive, coordinated care model to ensure high-quality patient outcomes. Acting as an extension of the primary care physician, the APP delivers both chronic and acute care services to patients. ESSENTIAL JOB FUNCTIONS The Advanced Practice Provider (APP) delivers patient care in accordance with their licensure and certification. Diagnoses and manages both acute and chronic conditions, including diabetes, hypertension, kidney disease, and other chronic illnesses, as well as minor injuries and acute medical issues such as infections or exacerbations of chronic diseases.
