The ideal candidate will work at the Deaconess IL Specialty Clinic (Family Clinic) in Mount Vernon, IL alongside an existing team consisting of one (1) Family Medicine provider, one (1) Pediatrics provider, and two (2) Advanced Practice Providers (APPs). Office hours are Monday through Friday, 7am to 3:30 pm. Provides clinical psychosocial intervention, including crisis intervention, individual, couples and family counseling, support groups, and short-term bereavement therapy. Coordinates patient care through the continuum, facilitating the achievement of optimal quality outcomes in relation to clinical care and cost-effectiveness.
