ChristianaCare's Medical Group is looking to hire a Full Time Psych Social Worker II t o support the 7N Inpatient Behavioral Health Unit at the Wilmington Hospital, DE. The 7N inpatient behavioral health unit has 30 beds with an average length of stay under 8 days. The Senior Psych Social Worker will assist patients and families to define social, emotional and environmental needs and to enable them to recognize/utilize available resources to resolve needs by providing a full spectrum of social work service. In addition, they will be responsible for the coordination of care, family therapy sessions, group therapy sessions, individual therapy sessions, safety planning, discharge planning, admission biopsychosocials, timely transmission of transition records, compliance with federal and state mandates for aftercare planning, treatment team, and support with involuntary commitments including court hearing.
