We are seeking to hire a Social Worker/Discharge Planner .
$23 to $27 per hour based on circumstances including skills, qualifications, degrees and certifications, prior job experience, training, market data, and other relevant factors.
Full-Time schedule.
Social Worker/Discharge Planner Qualifications:
Bachelor's degree in Social Work or a related field required.
MDS and Short-Term Discharge planning experience required.
Minimum of one year of Social Work in a healthcare setting.
Must have Skilled Nursing Facility (SNF)/Subacute experience.
