About this Job: General Summary of Position MedStar Health is looking for a Social Worker LICSW to join our team at MedStar House Call Program! As a Social Worker LICSW, you will screen and provide care coordination, psychosocial support to a panel of frail elders receiving home-based primary care. Leads outreach activities within community that align with overall MHCP initiatives. Member of an interdisciplinary team of providers including physician, nurse practitioner, and care coordinator responsible for ensuring outcomes aligned with goals of care for their patient panel. Trains and supervises entry level case managers according to MHCP patient census and resource allocation.
