About this Job: MedStar Health is looking for a Social Worker Master of Social Work (MSW) Per Visit to join our team at MedStar Health Home Care! The ideal candidate will be a licensed Social Worker with excellent clinical and communication skills. The Social Worker MSW Per Visit will provide social work services to MedStar Home Care patients and to Montgomery County contract clients with MedStar Home Care Partners. Provides patients with counseling and linkage with community services when appropriate and within the guidelines for service provision under home health regulations and/or policy and per Montgomery County contract requirements. Performs assessments, assists in the resolution of social problems impeding the patient's treatment, rehabilitation, and medical compliance, and assists other health team members in understanding the psychosocial and emotional factors related to the patient's health problems.
