Social Worker II - MSW - Behavioral Health; M-F - 8a-4:30p PRIMARY FUNCTION:   The Social Worker provides professional social work services to patients and families across Behavioral Health, Maternal–Infant, and medical settings. Responsibilities include psychosocial assessment, crisis intervention, discharge planning, counseling, and coordination of community resources. The Social Worker collaborates with interdisciplinary teams to promote safe transitions of care, optimize patient outcomes, and address social determinants of health.  PRINCIPAL DUTIES AND RESPONSIBILITIES: Behavioral Health Social Work  Conduct comprehensive psychosocial assessments for patients with mental health or substance-use conditions.  Provide crisis intervention, safety planning, de-escalation, and behavioral support.  Facilitate individual, family, and group therapeutic interventions.  Coordinate inpatient and outpatient psychiatric placements and community...
