Job Details Description Location/Department: Hospice  Job Summary: The Hospice Community Liaison is responsible for developing, nurturing, and maintaining relationships with physicians, hospitals, skilled nursing facilities, and community organizations to increase awareness and appropriate utilization of Hospice services. This role serves as a trusted resource for healthcare professionals, patients, and families by educating them on hospice philosophy, eligibility, and benefits while representing our organization with compassion, professionalism, and integrity.
