Behavioral Health Care Manager (LMSW/LCSW) – SNF | CoCM Program Overview: We are an established SNF physician group seeking a Behavioral Health Care Manager (BHCM) to support our Collaborative Care Model (CoCM) program across Skilled Nursing Facilities (SNFs) in Georgia. This role focuses on managing residents with depression, anxiety, and other behavioral health conditions through a structured, team-based model in partnership with primary care providers and a consulting psychiatrist. This is a high-impact, population health role centered on resident engagement, care coordination, and outcomes tracking.
