Job Summary The Supervisor Rehabilitation Therapy is a working team member who is also responsible for providing leadership and oversight to a team of rehabilitation staff, including physical therapists, occupational therapists, speech language pathologists, certified occupational therapy assistants, physical therapist assistants, rehab aides and other rehab team members. Provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. Promotes continuity of care and cost effectiveness through the integrating and functions of case management. Core Job Responsibilities Coordinates discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment.
