Overview Facilitates resource management activities for inpatients based on identified patients. Coordinates patient’s transition from hospital to home through collaboration with physicians, nursing staff and other healthcare professionals to enhance continuity of patient care, promote cost effective utilization of resources and enhance patient, nurse and physician satisfaction. Provides psychosocial assessment, resources and intervention to hospitalized patients. Qualifications Education:  Require BS degree in Social Work from an accredited college or university and current Alabama licensure. Experience:  Two years hospital experience preferred.
