Job Description
The Department of Family & Community medicine seeks a Licensed Clinical Social Worker (LCSW) to work as part of a multidisciplinary primary care team performing behavioral health assessments and counseling services and care coordination. The position will include the delivery of behavioral health counseling visits at a National Committee for Quality Assurance (NCQA) recognized level III Primary Care Medical Home (PCMH).
In addition to behavioral health counseling, a successful applicant will perform integrated behavioral health care case management services via coordinated care with primary care providers and clinic staff. A successful applicant must have the ability to obtain 3 letters of recommendation from peer LCSWs for credentialing purposes.

Schedule
Monday – Friday, 8:00am-5:00pm (40 hrs/week). Position requires attending quality improvement meetings on Wed mornings at 7:45AM, with corresponding offset in time off per week. Occasional quality improvement or management meetings at 7AM-8AM or 5pm-6pm m
