Family Nurse Practitioner, Community Health Primary Care
Location: Freeport, NY (On-site)
Job Type: Full-time, Direct Hire
Schedule: Monday to Friday, day shift
Pay: $128,000 to $145,000 per year
Deliver comprehensive primary care to adults and children at a community health center with fixed daytime hours and up to $50,000+ in loan repayment eligibility.
Must-Haves
- MSN with a Family Nurse Practitioner concentration from an accredited program
- Active, unrestricted New York State FNP license
- Current Healthcare Provider CPR/AED certification
- Proficiency with EMR software and Microsoft Office
- Knowledge of Medicare, Medicaid, and third-party billing requirements
- Strong communication and organizational skills
What You’ll Do
- Provide primary care and physical exams for pediatric and adult patients
- Order and interpret diagnostic tests and build treatment plans
- Prescribe medications, therapies, and specialist referrals
- Develop PCMH and Chronic Care Management care plans focused on goals and behavior change
- Document findings and care plans promptly in the EMR
- Coordinate with specialists, community programs, and social agencies
- Participate in chart audits and quality improvement with the CMO
- Lead in-service clinical training for staff
- Follow infection control and safety protocols
Nice-to-Haves
- 1 to 3+ years as an NP in family medicine or primary care
- Experience treating patients with intellectual and developmental disabilities (IDD)
- Bilingual English/Spanish
- Community health center experience
Compensation and Benefits
