Join a mission-driven primary care practice serving older adults in underserved communities. Provide comprehensive care in clinic and via telehealth, with a dedicated care team that helps address housing, nutrition, and transportation needs, so you can treat the whole patient.

HIGHLIGHTS

WHAT YOU'LL DO
- Provide full-spectrum primary care, in clinic and via telehealth
- Lead daily team huddles on care plans and priorities
- Partner with care team members on integrated patient plans
- Manage patient communication and follow-ups
- Coordinate with emergency departments on continuity of care
- Document in Athena EHR
REQUIREMENTS
- Must live within a 10-mile radius of Worcester, MA
- Active Massachusetts NP license, or ability to obtain before start
- 3+ years of primary care experience as a practicing NP or PA
- New graduates will not be considered
- Strong evidence-based primary care skills
- Comfort with EHR and digital health tools
- Ability to connect with patients from diverse backgrounds
- Strong communication and documentation skills
PREFERRED
- Experience with older adults in complex social settings
- Spanish proficiency
- Telehealth or remote patient monitoring experience
