• Schedule: Monday Thursday, 8:00 AM 5:00 PM; Friday, 8:00 AM 1:00 PM

• Coverage: Oct 9. Current active need is Oct 9, 15-16, Dec 3, 14, 23-24, Jan 4-7, 11-14, 18-21, 25-28, Feb 1-4, 8-11, 15-18, 22-25, March 1-4, 8-11, 15-18, 22-25, 29-31, April 1, 5-8, 12-16, 19-23, 26-30, May 3-7, 10-14, 17-217

• Practice: Aspen Dental

• Clinical Support: 3 Dental Assistants and 1 Registered Dental Hygienist

• Consistent coverage is preferred

• Aspen experience is a plus

The ideal provider should be comfortable performing a broad range of general dentistry procedures, including:

• Simple and surgical extractions

• Full-mouth extractions

• Denture procedures and denture steps

• Digital dentures

• Crown & bridge

• Comprehensive treatment planning

Providers should note any procedure limitations at the time of presentation.

• Active Indiana CSR required at the time of submission

• Active DEA required and must cover all applicable schedules

• Providers must meet all credentialing requirements prior to beginning the assignment
