JOB DESCRIPTION
- Provider must have experience in outpatient primary care and comfort working autonomously
- Well experienced in chronic disease management and able to have difficult conversations with patients regarding medication management and patient education
- Position to start ASAP for a (3) three month duration, with possibility of extension until permanent needs are met
