What You ll Do:

• Provide independent outpatient primary care services in a multidisciplinary team setting
• Deliver comprehensive medical and nursing care to individuals, families, and groups
• Emphasize health promotion, disease prevention, and wellness strategies
• Lead and collaborate with physicians, nurses, medical assistants, and staff to optimize patient outcomes
• Educate patients and families to support informed healthcare choices
• Maintain a safe, efficient clinical environment
• Support practice operations in a cost-effective manner
• Participate in ongoing professional development and continuing education

What You Need:

• Valid Nurse Practitioner (APRN) license for Vermont
• 1+ years of relevant outpatient primary care experience
• Strong communication, teamwork, and patient education skills
• Ability to work independently and as part of a coordinated healthcare team
