Locum Tenens Nurse Practitioner - Hospice & Palliative Care San Diego County, CA
Compassionate hospice and palliative care NP opportunity in San Diego County, California. We are seeking an experienced Nurse Practitioner for a locum tenens role serving patients throughout central and north San Diego County in home-based settings.
Job Details
- Location: San Diego County, California
- Setting: Home-based hospice and palliative care
- Start Date: ASAP
- Duration: 13 weeks
- Schedule: Monday-Friday, 8:00 AM-5:00 PM
- Shifts per Week: 5
- Shift Duration: 8-hour days
- Patient Volume: Approximately 3-4 patient visits per day, with a typical panel of about 15 patients
- Weekend Rotation: Rotating Sundays as needed, with a compensatory day during the week
Job Requirements
- Minimum 1 year of nursing experience in hospice care or palliative care
- Active California Registered Nurse license
- Active California Nurse Practitioner license
- Completion of an accredited Nurse Practitioner program and MSN or equivalent advanced nursing education
- National certification through AANP or ANCC
- Active DEA certification
- Active NPI number or ability to obtain promptly
- Current BLS/CPR certification
- Advanced Certification as a Hospice and Palliative Care Nurse (ACHPN) preferred; required within 1 year
- Valid California driver's license, reliable transportation, and proof of insurance
- Strong documentation and computer skills
- Bilingual Spanish skills preferred
Responsibilities
- Conduct comprehensive hospice and palliative care assessments in home settings
- Evaluate patient history, perform physical assessments, and develop individualized plans of care
- Collaborate with the interdisciplinary team, community physicians, patients, and families to coordinate care
- See new admissions to the hospice program and manage a small patient panel
- Provide education and counseling on disease progression, symptom management, medications, and available resources
- Facilitate goals-of-care discussions, advance care planning, advance directives, and POLST documentation
- Document patient encounters and care plans accurately within the electronic medical record
- Participate in interdisciplinary meetings, case conferences, and quality improvement initiatives
- Serve as a clinical resource and educator for staff on hospice and palliative care best practices
- Support rotating weekend coverage and occasional extended patient needs when necessary
