NP Cardiology - Heart Failure - opening in Albany, NY - Seeking a full time Nurse Practitioner to join a team specializing in Heart Failure care. - As a leading provider of cardiology services, we are committed to delivering compassionate, high-quality care to our patients. - This is an excellent opportunity for a motivated individual to work alongside experienced cardiologists and healthcare professionals in a collaborative, patient-centered environment. Overview: As a Nurse Practitioner in Heart Failure, you will play a key role in the management and care of patients with complex heart failure. - You will work closely with the cardiology team to assess, diagnose, and develop comprehensive treatment plans for patients, ensuring optimal outcomes. - This role requires a compassionate and knowledgeable individual who is passionate about heart failure management and improving patient quality of life.- Perform comprehensive patient assessments and diagnostic evaluations related to heart failure.- Develop and implement individualized care plans, including pharmacologic and non-pharmacologic interventions.- Monitor patient progress, adjust treatment plans as needed, and provide ongoing education to patients and families.- Collaborate with cardiologists, nurses, and other healthcare professionals to provide coordinated care.- Order and interpret diagnostic tests, including echocardiograms, EKGs, and lab work.- Participate in patient rounds, case discussions, and follow-up care.- Provide patient education regarding heart failure management, lifestyle modifications, and medication adherence.- Stay current with the latest research and advancements in heart failure care.- Document all patient interactions and maintain accurate and up-to-date medical records. Position Offers: Compensation is between $130,891-$150,933 per year. The posted salary reflects the starting range of total compensation and does not include productivity bonuses, incentives related to quality and performance, extra shift incentives, and other forms of cash compensation as applicable to the position. In addition, providers typically receive a CME allowance and other benefits offered by their medical group employer. Please note that this salary range is provided in accordance with State law and is subject to variation due to the factors noted above. Starting bonus of $15,000 as follows: $5,000 starting bonus. $5,000 retention bonus.
