Job Description
The Navigator position will coordinate with the Psych-Oncology team to remove patient barriers to accessing comprehensive cancer care in order to maximize patient health outcomes and psychosocial well being. The navigator will assist patients with accessing and completing distress screening tools through electronic health record as well as within mobile app.
This individual will provide practical support to cancer patients and address barriers to care throughout the patient’s continuum of care. The team member will participate in interdisciplinary care meetings and/or team meetings and document their interactions accordingly. The navigator will participate in research training and projects to foster patient and clinical team education regarding psychosocial support.

Qualifications
BA
5 yrs
Obtain approved national certification in area of specialty within 24 months.

Schedule
Monday- Friday, 40 hours per week, Variable schedule, will include evenings and offsite work
