Registered Nurse Case Manager

CVS Health is seeking a Registered Nurse Case Manager to provide field-based care coordination for medically complex members across New Jersey. In this role, you will assess patient needs, develop individualized care plans, coordinate services with providers, manage transitions of care, and support medication adherence. You'll educate patients and families, document accurately in electronic systems, and collaborate with interdisciplinary teams. This position requires regular community and in-home visits, strong clinical judgment, and a passion for improving outcomes and reducing hospitalizations.

Responsibilities

• Conduct comprehensive patient assessments in homes and community settings
• Develop and manage individualized care plans for complex patients
• Coordinate services with physicians, hospitals, and community providers
• Manage transitions of care to reduce readmissions
• Educate patients and families on conditions, medications, and self-management
