Responsibilities
- Develop and deliver community and patient health education programs and workshops.
- Assess community health needs and tailor education materials to diverse populations.
- Provide one-on-one and group teaching on prevention, chronic disease management, and wellness.
- Coordinate and participate in community outreach events, screenings, and health fairs.
- Collaborate with physicians, nurses, and allied health staff to integrate education into care plans.
- Evaluate program effectiveness, track attendance, and report key outcomes and metrics.
- Maintain accurate documentation in electronic health records and program databases.
- Ensure compliance with regulatory and organizational policies and standards of care.
- Serve as a clinical resource and advocate for community health initiatives.
- Participate in professional development and contribute to quality improvement efforts.
