Community Educator Nurse

Piedmont Healthcare seeks a Community Educator Nurse to deliver evidence-based health education to patients, families, and community groups. In this role, you will design and facilitate classes, screenings, and outreach events that promote prevention, chronic disease management, and wellness. You will collaborate with interdisciplinary teams, clinics, and local partners to assess community needs, develop culturally responsive materials, and track outcomes. Responsibilities

• Develop and deliver community and patient health education programs and workshops.
• Assess community health needs and tailor education materials to diverse populations.
• Provide one-on-one and group teaching on prevention, chronic disease management, and wellness.
• Coordinate and participate in community outreach events, screenings, and health fairs.
• Collaborate with physicians, nurses, and allied health staff to integrate education into care plans.
• Evaluate program effectiveness, track attendance, and report key outcomes and metrics.
• Maintain accurate documentation in electronic health records and program databases.
• Ensure compliance with regulatory and organizational policies and standards of care.
• Serve as a clinical resource and advocate for community health initiatives.
• Participate in professional development and contribute to quality improvement efforts.
