Job Description
Solaris HealthCare is seeking a Certified Physical Therapy Assistant to support rehabilitation services for seniors in our skilled nursing and long-term care community. In this resident-centered role, you will deliver treatments under the direction of a Physical Therapist, including therapeutic exercises, gait and transfer training, and mobility programs to improve strength, balance, and function. You will document progress in the EMR, educate residents and families, and collaborate with nursing and rehab teams. Responsibilities
- Provide rehabilitative treatments under the direction of a licensed Physical Therapist
- Implement therapeutic exercise, gait, transfer, and balance training programs
- Assist residents with mobility, functional tasks, and use of assistive devices
- Accurately document treatments, progress, and outcomes in the EMR system
- Support development and adjustment of care plans based on resident response
- Educate residents and families on home exercises, safety, and mobility strategies
- Collaborate with nursing, rehab, and interdisciplinary teams to coordinate care
- Adhere to state practice guidelines, company policies, and ethical standards
- Maintain a safe, clean treatment environment and follow infection control protocols
- Participate in team meetings, in-services, and ongoing professional development
