As a Transplant Coordinator at MEDSTAR HEALTH, you will manage the care continuum for organ transplant candidates and recipients across our regional system. You'll coordinate evaluations, listings, and post-transplant follow-up, collaborating closely with surgeons, physicians, nurses, social workers, and external partners. Responsibilities include patient education, data tracking, regulatory documentation, and timely communication to ensure safe, patient-centered care aligned with MedStar's mission of clinical excellence, innovation, and community service.
Responsibilities
- Coordinate pre- and post-transplant evaluations, listings, and follow-up care
- Serve as primary point of contact for transplant candidates, recipients, and families
- Collaborate with surgeons, physicians, nurses, and support staff on care plans
- Ensure compliance with UNOS, CMS, and hospital transplant regulations and policies
- Maintain accurate EMR documentation, registries, and required reports
- Educate patients and families on procedures, medications, and follow-up requirements
- Coordinate with organ procurement organizations and external providers
- Monitor patient status and escalate clinical concerns promptly
- Support quality improvement and outcome tracking initiatives
- Participate in on-call coverage and multidisciplinary transplant meetings
