Job Description
Adventist Health is seeking an Occupational Therapy Assistant to join our home health team in Clearlake. In this field-based role, you will provide one-on-one occupational therapy treatments in patients' homes, supporting recovery, safety, and independence in daily activities. Working under the supervision of an Occupational Therapist, you will implement care plans, document progress, and educate patients and caregivers. You'll collaborate closely with an interdisciplinary, faith-based team dedicated to whole-person care-mind, body, and spirit-while enjoying opportunities for growth and meaningful community impact.
Responsibilities
- Deliver occupational therapy treatments in patients' homes under OT supervision
- Perform patient assessments and document progress in EHREducate patients and caregivers on exercises, ADLs, and home safety
- Coordinate care with nurses, therapists, and other team members
- Travel within the Clearlake area to complete scheduled home visits
- Adhere to infection control, safety, and regulatory standards
- Support Adventist Health's mission and faith-based approach to care
- Participate in team meetings and ongoing professional development
