UCHealth is seeking a Care Manager RN to coordinate complex patient care across our Colorado-based health system. In this role, you'll use your clinical expertise to assess needs, develop and manage care plans, and ensure safe, timely transitions from hospital to home or post-acute settings. You'll collaborate closely with physicians, nurses, social workers, and community partners, while educating patients and families to promote self-management. Responsibilities
- Coordinate and manage patient care plans across the continuum from admission through discharge
- Perform RN-level clinical assessments to identify medical, psychosocial, and resource needs
- Collaborate with physicians, nurses, and interdisciplinary teams to optimize patient outcomes
- Arrange post-acute services, home health, durable medical equipment, and follow-up appointments
- Educate patients and families on diagnoses, medications, and self-care management
- Advocate for patients to ensure safe, timely, and cost-effective care transitions
- Monitor quality, utilization, and readmission risk, implementing interventions as needed
- Document assessments, care plans, and interventions accurately in the EMRParticipate in performance improvement and population health initiatives
- Precept or mentor staff in care management best practices and UCHealth standards
