Job Description
UCHealth is seeking a Diabetes Educator to join our Healthcare & Medical Services team. In this patient-centered role, you will provide evidence-based diabetes self-management education, develop individualized care plans, and support patients and families across inpatient and outpatient settings. Working closely with physicians, nurses, and care coordinators, you will deliver culturally sensitive education on medications, nutrition, technology, and lifestyle changes. Responsibilities
- Deliver individual and group diabetes self-management education using evidence-based standards
- Assess patient learning needs and develop individualized education and care plans
- Collaborate with physicians, nurses, dietitians, and pharmacists to optimize diabetes care
- Educate patients and families on medications, glucose monitoring, insulin administration, and devices
- Provide nutrition, exercise, and lifestyle counseling tailored to diverse populations
- Document assessments, education, and outcomes in the electronic health record
- Participate in quality improvement initiatives and population health projects
- Coordinate follow-up, referrals, and community resources for ongoing support
- Maintain current knowledge of diabetes guidelines, technology, and best practices
- Contribute to an inclusive, patient-centered, and safety-focused care environment
