Incredible Health is seeking a compassionate, detail-oriented Registered Nurse to deliver exceptional patient care within our partner healthcare & medical services network. In this role, you'll assess, plan, implement, and evaluate patient care, collaborate with interdisciplinary teams, and educate patients and families. You'll use evidence-based practices, follow safety and quality standards, and document accurately in electronic health records. Responsibilities
- Perform comprehensive and focused patient assessments and develop individualized plans of care
- Administer medications, treatments, and therapies safely and in accordance with provider orders and facility protocols
- Monitor patient status, interpret clinical data, and respond promptly to changes in condition
- Document all assessments, interventions, and outcomes accurately in the Electronic Health Record (EHR)
- Educate patients and families on diagnoses, medications, procedures, and post-discharge care plans
- Collaborate with physicians, advanced practice providers, and interdisciplinary team members to coordinate safe, efficient care
- Adhere to infection control standards, safety policies, and regulatory requirements
- Participate in quality improvement initiatives and contribute to optimizing clinical workflows
- Utilize digital and technology-driven tools provided through Incredible Health's platform to enhance communication, scheduling, and care coordination
- Support smooth patient transitions between levels of care and care settings, including discharge planning
