Job Description
BJC HealthCare seeks a Hospice RN Case Manager to deliver compassionate, patient-centered end-of-life care in home and community settings. In this field-based role, you'll manage a caseload of hospice patients, complete comprehensive assessments, develop and update individualized care plans, and coordinate services across an interdisciplinary team. You'll provide expert pain and symptom management, educate and support families, and ensure seamless communication with physicians and caregivers while documenting care accurately in alignment with BJC's standards of clinical excellence and mission-driven values.
Responsibilities
- Manage a caseload of hospice patients in home and community settings
- Conduct comprehensive nursing assessments and reassessments
- Develop, implement, and update individualized hospice care plans
- Provide pain and symptom management and monitor patient response
- Educate and support patients, caregivers, and families on disease process and end-of-life care
- Coordinate services and communication across the interdisciplinary hospice team
- Collaborate with physicians and other providers to adjust care plans
- Document all care, assessments, and communications in the EHR accurately and timely
- Participate in case conferences and quality initiatives to improve outcomes
- Ensure care aligns with regulatory standards and BJC Health
