Responsibilities
- Supervise and support RN case managers and case management support staff on the neurological unit
- Coordinate comprehensive discharge planning and transitions of care for neurology patients
- Collaborate with physicians, bedside nurses, social workers, and ancillary services to develop interdisciplinary care plans
- Conduct utilization review and monitor length of stay, readmission risk, and barriers to discharge
- Provide leadership in staffing oversight, scheduling, workflow coordination, and day-to-day case management operations
- Mentor, coach, and participate in performance evaluation and professional development of case management team members
- Ensure compliance with regulatory requirements, payer guidelines, and hospital policies related to case management
- Engage patients and families in care planning, providing education on post-acute services and community resources
- Participate in quality improvement initiatives and contribute to policy and protocol development for care coordination
- Serve as a clinical resource for case management best practices within the neurological unit
