Job Description
As a Case Manager RN at TriStar Health, you will coordinate safe, efficient care transitions for patients across our Middle Tennessee and Southern Kentucky network. You'll perform clinical assessments, create individualized care plans, and partner with physicians, nurses, and social services to support optimal outcomes, appropriate utilization, and timely discharge. You will educate patients and families, connect them with community resources, and advocate for their needs while upholding TriStar's patient-first, safety-focused, and evidence-based care standards.
Responsibilities
- Coordinate and manage patient care transitions across the continuum of care
- Conduct clinical assessments to develop individualized care plans
- Collaborate with physicians, nurses, and interdisciplinary teams to optimize outcomes
- Educate patients and families on treatment plans, resources, and discharge instructions
- Monitor patient progress, utilization, and length of stay to support quality and cost-effective care
- Advocate for patients to ensure access to appropriate services and community resources
- Document assessments, plans, and outcomes in the electronic health record
- Support quality, safety, and evidence-based practice initiatives
