TriStar Health is seeking an RN Case Manager to coordinate patient care within our Healthcare & Medical Services department. In this full-time role, you will perform comprehensive assessments, develop and manage discharge plans, and guide patients and families through the continuum of care. Responsibilities
- Coordinate comprehensive care plans for patients across the continuum of care
- Conduct clinical assessments and develop individualized discharge plans
- Collaborate with physicians, therapists, and social workers to ensure safe transitions
- Facilitate access to community resources, home health, and post-acute services
- Perform utilization review and support appropriate level-of-care decisions
- Educate patients and families on care plans, medications, and follow-up needs
- Document assessments, plans, and outcomes in the EMR accurately and timely
- Support quality improvement initiatives and adherence to clinical protocols
- Advocate for patients while balancing clinical and resource stewardship
- Participate in interdisciplinary rounds and case conferences
