Family Nurse Practitioner

Fast Pace Health is seeking a compassionate Family Nurse Practitioner to deliver primary and urgent care services in our rural community clinic. In this role, you will provide comprehensive assessments, diagnose and treat acute and chronic conditions, order and interpret diagnostics, and educate patients across the lifespan. Responsibilities

• Provide comprehensive primary and episodic care to patients across the lifespan in a clinic setting
• Conduct health assessments, diagnose illnesses, and develop evidence-based treatment plans
• Order, interpret, and follow up on diagnostic tests and imaging
• Manage chronic conditions such as diabetes, hypertension, and COPDDeliver urgent care services for minor injuries and acute illnesses
• Educate patients and families on disease prevention, medications, and healthy lifestyles
• Collaborate with physicians, behavioral health, and diagnostic teams to coordinate care
• Document all patient encounters accurately in the EHR and ensure regulatory compliance
• Participate in quality improvement, clinical protocols, and care standardization initiatives
• Contribute to a culture of compassion, innovation, and community-focused care
