Family Nurse Practitioner

Fast Pace Health seeks a compassionate Family Nurse Practitioner to deliver high-quality primary and urgent care to patients across the lifespan in our rural community clinics. In this role, you will assess, diagnose, and manage acute and chronic conditions, order and interpret diagnostic tests, prescribe medications, and provide preventative and wellness care. Responsibilities

• Provide comprehensive primary and urgent care to patients across the lifespan in a clinic setting
• Conduct assessments, diagnose conditions, and develop evidence-based treatment plans
• Order, interpret, and act on labs, imaging, and other diagnostic tests
• Prescribe medications and manage chronic diseases in alignment with clinical guidelines
• Deliver patient and family education focused on prevention and wellness
• Collaborate with physicians, nurses, and allied health professionals to coordinate care
• Document all encounters accurately in the EMR/EHR system
• Support quality, safety, and performance improvement initiatives within the clinic
• Participate in community-focused care consistent with Fast Pace Health's mission
• Maintain required licensure, certifications, and continuing education
