Acute Care Nurse Practitioner

Job Description

Memorial Hermann Health System seeks an Acute Care Nurse Practitioner to deliver high-quality, evidence-based care to hospitalized patients in a fast-paced acute care setting. In this role, you will assess, diagnose, and manage complex conditions, order and interpret diagnostic tests, perform approved procedures, and develop individualized treatment plans. You will collaborate closely with physicians and interdisciplinary teams, support safe transitions of care, document thoroughly in the EHR, and contribute to quality and safety initiatives in an innovative, inclusive environment that supports professional growth.

Responsibilities

  • Provide comprehensive assessment, diagnosis, and management for acute and complex hospitalized patients.
  • Order, interpret, and act on diagnostic tests, labs, and imaging to guide treatment.
  • Perform approved procedures and interventions within NP scope and hospital privileges.
  • Develop and adjust evidence-based care plans in collaboration with physicians and interdisciplinary teams.
  • Document thoroughly and accurately in the EHR, ensuring continuity and quality of care.
  • Educate patients and families on conditions, treatments, and discharge plans.
  • Participate in quality improvement, safety initiatives, and clinical protocols.
  • Collaborate with nursing, pharmacy, therapy, and case management to optimize patient outcomes.
  • Support transitions of care, including discharge planning and follow-up recommendations.
  • Precept and mentor nurses or students, contributing to a collaborative learning culture.
