Responsibilities
- Coordinate and deliver comprehensive renal med-surg nursing care for a diverse adult patient population
- Lead daily patient care activities, prioritizing assessments, treatments, and interdisciplinary communication
- Serve as a clinical resource and preceptor to nursing staff, supporting orientation and ongoing development
- Develop, implement, and evaluate individualized care plans focused on optimal patient outcomes
- Ensure accurate EHR documentation, medication safety, and adherence to evidence-based protocols
- Collaborate with physicians, therapists, and case management on complex discharges and transitions of care
- Educate patients and families on renal disease, medications, and post-discharge self-management
- Support quality improvement, infection prevention, and patient safety initiatives on the unit
