Responsibilities
- Conduct comprehensive evaluations of speech, language, cognitive-communication, and swallowing disorders.
- Develop and implement individualized, evidence-based treatment plans aligned with patient goals and SSM Health standards.
- Collaborate with physicians, nurses, therapists, and caregivers to coordinate holistic, patient-centered care.
- Document assessments, progress notes, and treatment outcomes accurately in the electronic medical record.
- Educate patients and families on strategies to support communication and swallowing at home and in the community.
- Participate in interdisciplinary meetings, care conferences, and quality improvement initiatives.
- Provide mentorship and support to students or new team members as appropriate.
