Westview Health Care Center in Dayville, CT seeks an LPN Licensed Practical Nurse to provide compassionate, resident-centered care in a skilled nursing and rehabilitation setting. In this role, you'll deliver hands-on clinical care, administer medications and treatments, monitor resident conditions, and document accurately. You'll collaborate with an experienced interdisciplinary team, support residents in short-term rehab, long-term care, and memory care, and help maintain high standards of quality, safety, and dignity in a supportive, community-focused environment with opportunities for professional growth.
Responsibilities
- Provide direct nursing care to residents, including medication administration, treatments, and monitoring vital signs
- Support and implement care plans in collaboration with RNs, physicians, and the interdisciplinary team
- Document resident status, interventions, and responses in accordance with regulatory and facility standards
- Assist with admissions, discharges, and transitions of care, ensuring resident and family understanding
- Respond promptly to changes in resident condition and report concerns to supervising nurse or provider
- Educate residents and families on care routines, medications, and health maintenance
- Promote resident dignity, safety, and comfort in all aspects of daily care
- Maintain infection control, safety, and quality standards in the skilled nursing environment
- Supervise and support CNAs and other support staff in delivering high-quality care
- Participate in quality improvement, trainings, and team meetings to enhance clinical practice
