Occupational Therapist Home Health

CenterWell Home Health is seeking an Occupational Therapist to deliver compassionate, in-home therapy to seniors and adults with chronic conditions. In this field-based role, you will conduct comprehensive assessments, create individualized treatment plans, and provide hands-on interventions that improve safety, function, and independence at home. You'll collaborate closely with nurses, therapists, physicians, and caregivers, and document care in accordance with clinical and regulatory standards. Responsibilities

• Provide in-home occupational therapy evaluations and treatments for adult and senior patients
• Develop and implement individualized care plans to improve safety, function, and independence at home
• Collaborate with nurses, therapists, physicians, and caregivers as part of an interdisciplinary team
• Educate patients and families on adaptive techniques, home safety, and use of assistive devices
• Document assessments, treatment plans, progress notes, and outcomes in accordance with regulatory and agency standards
• Participate in care conferences and contribute to ongoing care coordination
• Monitor patient progress and adjust interventions and goals as needed
• Ensure compliance with clinical, ethical, and safety guidelines while working independently in the field
